Schedule Appointment

Please read the following information so that we can provide you with the best possible care.

We accept most insurance plans including Medicare. If you have any questions about our
participation with your insurance plan, please call our office or your insurance company before
scheduling an appointment.

If your insurance plan requires a referral to be seen by a Specialist, please obtain it from your
Primary Care Doctor BEFORE your visit. If we do not have your referral by the time of your
appointment, it will need to be canceled.

Registration
Online appointment scheduling is available for medical office visits only. If you are interested in
scheduling a cosmetic appointment, please call the office directly.

Be sure to use exactly the same name to register that is printed on your insurance card.

ESTABLISHED PATIENTS: After entering your name and date of birth, you will see “welcome
back!” appear next to your name and prompted to confirm your automatically populated
information. IF YOUR INFORMATION DOES NOT AUTOMATICALLY POPULATE, PLEASE CALL THE OFFICE TO SCHEDULE YOUR APPOINTMENT. DO NOT CONTINUE TO CREATE A NEW PROFILE TO AVOID DUPLICATING YOUR ESTABLISHED ACCOUNT.
 
After scheduling an appointment, you will receive a registration link from eDerm.
If you do not receive the email, please check your Spam Folder. If you are unable to find the
email, please call the office.
 
New Patients:
Registration forms can be found on our website home page under the New Patients link. We
ask that you print and complete these forms and bring them with you at the time of your visit.
We kindly ask that you arrive 15 minutes prior to your appointment time for registration. Please
bring your photo ID, insurance cards (including prescription cards), a referral if required by your insurance, and a list of your current medications.
